All students entering grades 7-12 interested in participating in athletics and/or marching band must complete a pre-participation physical exam every year.  Students interested in athletics and/or marching band are also required to complete online forms through Rank One.  The pre-participation physical and online forms must be completed prior to participation in athletics. 


Please note that minor students must have a parent/legal guardian present when receiving their physicals. The Walk-In Clinic is open 7 days a week from 10:00 am to 6:00 pm and is located at 202 S McGee St in Borger. Incoming 7th graders and those new to athletics must receive a Pre-Participation Physical Exam and complete online forms prior to participation in BEAST Camp over the summer. Also, note that the hard copy of the Medical History/Pre-Participation Physical Exam must be turned in to Coach Ortiz or the Athletic Department. This must be signed by both the student and parent/legal guardian.
